Hypertrophic Scars

(Red or Dark and Raised)
When a normal wound heals the body produces new collagen fibres at a rate which balances the breakdown of old collagen. Hypertrophic scars are red and thick and may be itchy or painful. They do not extend beyond the boundary of the original wound but may continue to thicken for up to 6 months. They usually improve over the next one to two years but may cause distress due to their appearance or the intensity of the itching, also restricting movement if they are located close to a joint.

Hypertrophic scars are more common in the young and people with darker skin. Some people have an inherited tendency to this type of scarring. It is not possible to completely prevent hypertrophic scars, so anyone who has suffered one should inform their doctor or surgeon if they need to have surgery. Scars do not occur in younger people as often as older people because their skin cells replicate more quickly and fill in the wound with normal skin tissue.

Although the scars are innervated with blood vessels, they lack the oil glands and elastic tissue that normally protect the skin against irritation and can be painful or itchy. When these types of scars cover wide areas of skin, especially from massive wounds or burns, they can make movement difficult as well.

Over-scarring also occurs in cases where the healing process doesn't complete the "remodeling" process properly. During this normal phase of healing the body adds more collagen to the scar for added skin strength and then it should start removing portions of the same collagen over time, continually shrinking the scar. Sometimes this does not happen as it should and the result is excessive scarring. 

Keloids are scars that are excessively thick and fibrous, tumour-like growths that extend beyond the wound's original limits. While not dangerous, keloid scars may itch, cause pain or be tender to the touch. Over time, they can develop claw-like projections into the surrounding skin. In the worse cases, they can be quite disfiguring.

Keloids usually originate from an injury such a burn or severe acne, but they can also arise from minor injuries as a scratch or an insect bite. While the skin begins healing normally, something goes awry and the scar overfills with collagen, causing it to swell and enlarge.

No one understands why keloids form and while surgery (traditional and cryosurgery) can remove keloids, they often return. Whatever the cause, there does appear to be a genetic component. This type of scarring is found most often in people of African and Asian origins and also seems to run in families.
Scars and keloids do not usually cause medical problems, but they are unsightly and unappealing from an aesthetic point of view.
