Surgical Methods

Surgery is a form of artificial skin repair. A common side effect of surgery is the scar that may be left behind by the procedure. Sometimes these simple scars can transform into keloids, and surgery may again be needed to remove the scar tissue. Sun damaged skin sometimes needs to be removed by surgical procedures to insure the melanoma has been completely removed. In this page, we will review cosmetic and medical surgeries.

Surgery

Surgery is defined traditionally as treatment that involves cutting or stitching tissue. However, advances in surgical techniques have made the definition more complicated. Sometimes lasers, rather than scalpels, are used to cut tissue, and wounds may be closed without stitches. Surgery is one of the oldest forms of cancer treatment and often the most effective. Surgery is usually the sole or primary means of treating tumors that have not spread beyond their original site of growth, however this technique cannot be used for all early-stage cancers.


Cosmetic Surgery

Involves a wide variety of operations, including removing facial and neck wrinkles (rhytidectomy); removing fat and wrinkles from the abdomen (abdominoplasty); enlarging or reducing breasts (mammoplasty); restoring scalp hair (hair replacement surgery); altering the appearance of facial features, such as the jaw (mandibuloplasty), eyelids (blepharoplasty), and nose (rhinoplasty); removing body fat (liposuction); and eliminating varicose veins (sclerotherapy). Surgeries like these require close adherence to instructions after the operation. It needs to be understood that the results from cosmetic surgery may not be exactly what the person is expecting so issues regarding appearance and scars should be discussed in advance with the surgeon performing the procedure. 

Major and Minor Surgery: A distinction is sometimes made between major and minor surgery, although many surgical procedures have characteristics of both.

Major surgeries usually involve the use of general anesthesia. Major surgeries often involve opening one of the major body cavities, such as the abdomen, chest, or skull.

Minor surgery can involve the use of local, regional, or general anesthesia. Major body cavities are not opened. Minor surgery may be performed in an emergency department, an ambulatory surgical center, or a doctor's office. Vital organs usually are not stressed as with a major surgery and normally the surgery can be performed by a single doctor.

Without distinguishing between minor or major surgery, we will consider just three types of deformities from a surgical standpoint and its procedures of healing: keloids scars, skin grafts, and from the procedures to extract a skin cancer (melanoma and others).

Keloid scars are actually thick, puckered, itchy clusters of scar tissue that grow beyond the edges of a wound or incision and rarely regress. Keloid scars are sometimes very nodular in nature, and they are often darker in color than surrounding skin. They occur when the body continues to produce tough, fibrous protein (known as collagen) after a wound has healed.

Keloids are fibrotic tumors characterized by a collection of atypical fibroblasts with excessive deposition of extracellular matrix components, especially collagen, fibronectin, elastin, and proteoglycans. Histologically, keloids contain relatively acellular centers and thick, abundant collagen bundles that form nodules in the deep dermal portion of the lesion. Keloids present a therapeutic challenge that must be addressed as these lesions can cause significant pain, itching, and physical disfigurement. They may not improve in appearance over time, and can even limit mobility if located over a joint.

Skin Graft method is used to replace burned skin on people who have lost large areas of skin (usually deep minor burns). Other skin grafts help by temporarily covering and protecting the skin as it heals on its own. In a skin grafting procedure, a piece of healthy skin is taken from an unburned area of the person's body (autograft), from another living or dead person (allograft), or from another species (xenograft)—usually pigs because their skin is most similar to human skin. The skin graft is surgically sewn over the burned area after removing any dead tissue and ensuring that the wound is clean. Autografts are permanent. Allografts and xenografts, however, are rejected after 10 to 14 days by the person's immune system. Artificial skin has been developed recently and can also be used to replace the burned skin. Burned skin can be replaced anytime within several days of the burn.

Skin Cancer is the most common form of cancer in the United States. The three main types of skin cancer—basal cell carcinoma, squamous cell carcinoma, and melanoma—are caused, at least in part, by long-term sun exposure. Surgery is one of the oldest forms of cancer treatment and often is the most effective. Surgery is usually the sole or primary means of treating tumors that have not spread beyond their original site of growth. However, surgery cannot be used for all early-stage cancers. Usually, the defect that is left in the skin is small. Larger or more invasive cancer may require removal of a significant amount of skin, which may have to be replaced with a skin graft.

Scars from Cosmetic Surgeries

The scars from cosmetic surgeries are placed in areas where they are the least noticeable. Every surgery leaves a different scar.

Breast enlargement: implants are inserted through an incision-in your underarm, at the edge of your nipple, or in the under crease of your breast. Scarring, in addition to being unsightly, may lead to discomfort from the implant.

Breast lift: incisions are made around the nipple and under the breast; if your breasts are very large, an extensive anchor-shaped incision may be necessary and will leave a large scar.

Breast reconstruction: the degree of scarring will depend on the extent of the surgery you require. Quite radical breast reconstruction may be needed after a mastectomy.

Breast reduction:
the incision site lies below the nipple toward the bottom of the breast.

Tummy tuck: skin and fat are removed from between your navel and pubic hair. Scarring is inevitable but fades over time.

Liposuction: the small incision may be any place you feel you have excess fat-thighs, tummy, buttocks, upper arms, neck etc.
