Cosmetic Laser Surgery
Laser skin procedures are related to other kinds of skin resurfacing techniques such a chemical peels and dermabration. We will look at laser skin procedures, its treatment, how it affects the skin, time of recovery, and who is the best candidate.
Laser or Light Amplification by the Stimulated Emission of Radiation works by producing an intense beam of bright light that travels in one direction. This laser beam can cut, seal, or vaporize skin tissue and blood vessels. In fact, lasers can actually coagulate tissue in order to stop bleeding. The laser has the unique ability to produce one specific color (wavelength) of light which can be varied in its intensity and pulse duration. Ordinary light from non-laser sources is composed of many different colors and appears white. The wavelength and power output of a particular laser determines its medical application.
Since their 1958 discovery, lasers have become a powerful industrial tool. Their application in medicine has been truly revolutionary. One reason, says Richard Felten, a senior reviewer in FDA's Center for Devices and Radiological Health, is that lasers used as surgical tools can cut through tissue without causing excessive bleeding.
Laser Skin Resurfacing
Laser resurfacing is a very controlled burning procedure during which a laser vaporizes superficial layers of facial skin, removing not only wrinkles and lines caused by sun damage and facial expressions, but also acne scars, folds, and creases around the nose and mouth. Lasers can even help precancerous and benign superficial growths. In a sense, the laser procedure creates a fresh surface over which new skin can grow.
Laser skin resurfacing offers a number of advantages over more traditional techniques, such as dermabration or chemical peels. There is virtually no bleeding and, often, no scarring. It can also be used in combination with traditional surgical techniques -- such as an eyelift or facelift -- to improve specific areas of the face that may not be improved by surgery alone.
Laser resurfacing can often make patients look 10 to 20 years younger, and the results can last for eight to 10 years, says Tina Alster, M.D., director of the Washington Institute of Dermatologic Laser Surgery in the nation's capital. But she warns that after surgery, patients must avoid sunbathing and destroying their skin again. Usually the first procedure is so successful that a follow-up is not needed.
Laser Skin Resurfacing Risks
Alster warns that anyone not mentally prepared for resurfacing or who expects instant results is not a good candidate. "This is not easy in-easy out surgery," she says. "Potential patients have to realize that there will be bruising and swelling and they will be holed up in the house for seven to 10 days. Certain people with very sensitive skin cannot tolerate the medications and lubricants used on the skin during healing.” Darker-skinned ethnic groups are not candidates for laser skin resurfacing because the laser treatment alters the color of skin too dramatically and unpredictably.
As with any medical procedure, patients may experience certain complications--most temporary--including a prolonged redness of skin, tenderness, easy flushing, and some pigment changes, like hyperpigmentation, when the skin appears darker than normal, says Rox Anderson, M.D., director of the Laser Center at Massachusetts General Hospital in Boston.
Other risks are more serious, and possibly permanent, including hypopigmentation, or lightening of the skin. "Somewhere between one to two years after treatment it becomes clear that there is a permanent lightening of the skin color where the resurfacing was done," Anderson says.
"The person planning to do laser surgery must understand the basic physics of how laser energy is absorbed by tissue and how tissue responds," warns FDA's Felten.
Some other risks are scars directly related from a skin regeneration system or depending on the cause of laser skin procedures. For example, to remove a skin cancerous tumor, skin pigmentation changes and herpes breaks out if the patient is susceptible to such infections.
